All fellowship applicants must submit the following items to an AFFAM-fellowship program prior to being eligible for consideration:
Upon receipt of these items, applicants will be considered for an in-person interview at the request of the fellowship program director. The fellowship program will establish its choice of applicant 6 months prior to the fellowship start day.
Fellowships will run bi-annually, with the Spring program occurring from January 1st through June 30th and the Fall program occurring from July 1st through December 31st. Fellowship applications must be received by the following deadlines:
Spring Program – by May 31st of preceding year.
Fall Program – by November 30th of preceding year.
Fellowships are awarded 6 months prior to their respective start dates.
Fellows are required to complete the following coursework and practical caseload requirements in order to become eligible to receive a certificate of completion:
Fellowship requirements must be met within 6 months of fellowship completion or eligibility for certification will be voided.
Fellows must complete a research project in the subject of aesthetic medicine, which they will present at the conclusion of the fellowship. Fellowship projects may be one of the following:
Fellows are required to take and pass a 100-question written examination on the principles of aesthetic medicine contained within the fellow reading list. A passing score is 80% or higher. Fellows may retake the examination once and must obtain a passing score to receive certification.
At the completion of the fellowship, the fellowship director shall review and evaluate the fellow’s performance and clinical competency based on fellow’s conduct, performance, and satisfaction of fellowship requirements. Director will outline any areas in need of further improvement and recommend any remedial actions prior to recommending fellow for certification. The fellow’s performance is evaluated in six different core areas of competency:
1. Interpersonal and Communication Skills
2. Systems-based Practice
3. Practice-based Learning and Improvement
4. Professionalism
5. Patient Care
6. Medical Knowledge.
Treatment Case Log
Fellows are required to complete a minimum of 100 supervised patient treatments incorporating neuromodulator, fillers, skin resurfacing, and radiofrequency treatment modalities during fellowship. All treatments performed by the fellow during the course of the fellowship must be supervised by a fellowship instructor/director, entered into the case log, and signed off by the supervising provider. Fellow may take up to additional 6 months to complete the 100-case requirement. Failure to complete this requirement will result in ineligibility to receive certification.

Fellows are required to observe the minimum-required relevant surgeries/procedures prior to receiving fellowship certification:
- Rhytidectomy
- Rhinoplasty
- Upper blepharoplasty
- Lower blepharoplasty
- Browlift
- Chin augmentation
- Fractional CO2 laser skin resurfacing
- Radiofrequency skin tightening
Fellows are required to complete a fellowship evaluation at the end of the fellowship period. The evaluation is a requirement prior to receiving a certificate of completion.
